Asthma: If your student has asthma, please have their provider complete the forms in the Asthma Packet each year before the start of school.

Dental Form

Diabetes: If your student has diabetes, please have their provider complete the forms in the Diabetes Packet each year before the start of school. 

Head Lice Information

Medication Policy and Form - please have this filled out by a provider if your student requires any medications during school hours. Parent or guardian must also sign the bottom and bring in the medication to be administered. 

Physical Examination Form

Seizures: If your student has a seizure disorder, please have their provider complete the forms in the Seizure Packet each year before the start of school.
